ICU at Home
Two weeks ago we were approached by a family who wanted to shift their family member home from the ICU of a major tertiary care hospital.
She had already spent two months in ICU.
We connected with her intensivist in the hospital.
This is our regular practice to be able to ensure continuity of care.
He informed me that the patient needed full fledged critical care services.
She had a tracheostomy, was not able to maintain saturation without a full fledged ventilator.
She was on IV antibiotics, Vasopressors through a syringe pump and dialysis every alternate day.
She would need regular doctors visits and of course round-the -clock nursing care.
Since they were very sure they wanted to move home, we informed them of the limitations of home based care as against hospital based care.
So the complete ICU set up was established at home.
Ventilator was to give a trial with our ventilator on settings decided by intensivist and respiratory therapist in the hospital.
Nurse joined duty in the hospital to ensure smooth transition. She accompanied the patient when she was shifted by cardiac ambulance to home.
Doctor visited the patient the same evening and stabilised her.
It has been 15 days as I write this piece.
The patient is still in our care. It has not been an easy course but we have managed.
This entire experience has shown us, we have enhanced our capabilities over time.
I am also proud of our team for doing a stellar job under very challenging circumstances!
We now have the confidence to serve many more patients with full -fledged Critical Care at home.
